Community health in Clark, Klickitat, and Skamania counties

At Southwest Washington Accountable Community of Health (SWACH), we work to make health care more fair and accessible for everyone in Clark, Klickitat and Skamania counties. Our work is rooted in collaboration, ensuring that community health services and whole-person wellness are accessible to all—whether you live in a city or a rural area.

We interact with a handful of audiences in this work. We regularly collaborate with the local Community-Based Workforce. Our goal is to support them in their work, connect them to the resources and information they need, and provide occasional trainings and shared-learning.

Connecting with the Community-Based Workforce

One of our main priorities is supporting the Community-Based Workforce (CBW). We work alongside Community Health Workers (CHWs), peer support specialists, and other frontline professionals by:

  • Offering trainings and shared-learning opportunities.

  • Providing access to important resources and health information.

  • Advocating for policies that strengthen community-based care.

When we support the people serving on the frontlines, communities get the care and connections they need to thrive.

Strengthening Partnerships for Better Health

We partner with nonprofits, health care organizations, and local agencies that that work to improve health care and social well-being. As both a funder and a convener, SWACH helps partners:

  • Align their work toward shared health goals.

  • Secure funding and resources for community health projects.

  • Develop programs that address the root causes of health inequities.

We also collaborate with other Accountable Communities of Health (ACHs) across Washington, and with state agencies like the Department of Health and Health Care Authority. Together, we work for long-term, systemic improvements in health care.

True health equity means everyone can access the care, services, and resources they need to live healthy lives. Our work focuses on:

  • Addressing systemic inequities and social factors that affect health.

  • Removing barriers to care.

  • Advocating for policies that promote fairness and equity in health care.
