What is HealthConnect Hub?

SWACH’s HealthConnect Hub is a network that connects people and families across Southwest Washington to the care and resources they need, when and how they need them. Through a team of Community-Based Workers, we support community members and advocate for social and health equity. The Hub is part of the larger Community Care Hubs of Washington network, which helps coordinate groups of social and health services across the state, making it easier for people to access support. Our goal is to build healthier, more resilient communities throughout the region.

Do you need support accessing health or social services? We can help! SWACH can connect you with a Community-Based Worker (CBW) to help you with things like accessing a primary care physician, managing chronic conditions, and even affordable housing, food, and more. To get started, fill out a short form and let us know what assistance you need.

Southwest Washington Accountable Community of Health’s (SWACH) HealthConnect Hub leads efforts to help people achieve their health goals through tailored programs. Collaborative partnerships promote health equity and help sustain the overall goal of whole-person wellness.
