Community Partner Survey

At SWACH, we strive to center community voice and needs in our decision-making. Our partners’ feedback has the power to make remarkable change across our organization. Each year, we invite partner organizations to complete the Community Partner Survey - which helps us better understand what is working well, how we can improve, and what our community partners need to be successful.

The Community Partner Survey can be completed in as little as five minutes. Your feedback will help us understand how effectively we are collaborating with your organization, highlight successes and risks of our programs, and help us determine what community needs we need to address.
