HealthConnect: Fall 2020 Update
New Community Care Coordinators! New Connections!
In November, HealthConnect will be adding twelve additional community care coordinators.
The number of housing agencies partnering with HealthConnect is also increasing, from three to five. Those agencies now include: Council for the Homeless, Share, Vancouver Housing Authority, Outsiders Inn, and Washington Gorge Action Programs (WAGAP).
Continuing the expansion of HealthConnect Pathways partnerships increases capacity and the number of referrals that the community-based workforce (community care coordinators) are managing.
Hep C Cures + Outreach + CCOT = More People Reached; More Lives Impacted
The Hepatitis C Cures program is developing an outreach effort to reach more of those who could benefit. Peers will be working to connect with and support these people. COVID has had an impact on the number of participants going through the program. Connecting with these folks, and making the program virtual, will improve accessibility.
Clark County Opioid Task Force (CCOT) will be hosting the Hep C Cures group this Friday, October 23rd from 2-4p. Natalie and Hep C will be joining the CCOT meeting this week. Hep C Cures and the CCOT have overlap in the folks they serve, so this will be an important convening of like-minded groups, peers, and support for where people are.

SWACH’s HealthConnect HUB offers a continuing education opportunity for community-based care coordinators to share and learn common principles and practices of CHWs and Peers.
The training is offered virtually using a popular education framework- a highly effective strategy for sharing information, building skills and developing leadership. Facilitators are Peer Supporters, CHWs, and experienced educators and developers of CHW and Peer workforce development opportunities.
Participation in this continuing education opportunity is offered free of charge and requires a commitment to participate in all eight sessions. This is an opportunity to share, learn and strengthen your networking connections with a cohort of community-based care coordinators in our region.

Additionally, we are excited that the separate, upcoming HealthConnect Expansion training filled up! We are excited to continue offering training and develop for this growing workforce of Community-based Care Coordinators, including Community Health Workers and Peers.
