SWACH Project Plan Portfolio Complete!

All 219 singly spaced pages of our Medicaid Transformation Project Plan have been launched out into the real world. We owe a huge debt of thanks to our dozens of collaborators - from doctors and social workers to the Sheriff's department, community organizations and truly too many people to mention. Our portfolio submission documents, among other things, why we prioritized the four projects we chose from among eight approved by the state. For each project we explain how we will make sure to work with, and not duplicate, existing programs and how our efforts will build community capacity and generate a return on investment. We encourage you to read or skim the project plan  for important information including charts that show at a glimpse some of our local challenges and opportunities. Myers and Stauffer, the state's independent assessor, will review the plans of all nine ACHs across the state. That process wraps up in early February at which time full implementation begins!
